New patients have three short forms to complete before their first appointment. They take about 15 minutes total and need to be submitted at least 24 hours before your visit — otherwise we'll need to reschedule.

How to join your visit.
- Your Google Meet link will be sent to you by text and email
- At appointment time, just click the link from your text or email
- Click in a few minutes early
- Test your camera, mic, and Wi-Fi ahead of time
- Find a quiet, private space
- 700 Camp St, New Orleans, LA 70130
- Street parking available; paid lot at 732 Camp St on the same block
- Enter through the front door on Camp St and have a seat in the lobby
- Crystal will come down to greet you at your scheduled time

Patients who spend 5 minutes with our prepare tool consistently get more from their appointment. It walks you through what's actually happening in your body, why your symptoms are real, and what to expect from the conversation. Especially helpful if this is your first visit.
