After Your First Visit

Your visit gave you answers. Membership is how your care continues.

At your first visit, your provider talked through your symptoms, your history, and the treatment that fits you. Membership keeps that care going between visits — messaging, refills, dose adjustments, and lab review.

1
Fill out one enrollment form — about 5–8 minutes, covers your registration, billing authorization, lab preferences, and a photo ID upload.
2
Set up your patient portal — account and payment method. About five minutes.
3
Your provider messages you in the portal within 24 hours (Mon–Fri) with your treatment plan and prescriptions.
Enroll within 2 weeks of your visit and your visit fee credits toward month one — you'd pay $79 (video) or $29 (in person) to start, instead of full price.

Care that keeps going — no waiting room required.

Most ongoing care happens through messaging in the patient portal — no scheduling, no waiting room, no copays per visit. Video visits are available when you need them.

Message your provider anytime.

Refills, dose changes, side effects, lab results — message us in the portal whenever something comes up. No appointments needed for most things.

A real response within 24 hours, M–F.

No appointment needed — message your concern in the portal and your provider replies personally with prescriptions, education, and clear next steps.

Same-day prescriptions to your pharmacy.

When your provider writes a prescription, it's sent electronically to your pharmacy the same day. Pickup timing is up to your pharmacy.

Why is care membership-based?

Insurance reimburses for visits — the appointment itself. It doesn't reimburse the messaging, refill management, lab review, and dose adjustments that actually deliver your treatment between visits. Membership is what funds that ongoing care so your provider can give you real attention without the constraints of fee-for-service billing.

What's included with every membership
  • Your written treatment plan
  • Prescriptions to your pharmacy
  • Lab orders & review
  • Dose adjustments as you respond
  • Refill management
  • Patient education handouts
  • HSA / FSA documentation upon request
  • Superbill for insurance reimbursement

Care that actually moves the needle.

The goal isn't that you were seen — it's that you sleep again, think clearly, and feel like yourself. Here's what that looks like for the women who continue care with us.

Your membership, start to steady.

Ongoing care is a flat monthly membership — no per-visit copays, no surprise bills. Everyone starts in Active Care while we get your treatment working, then steps down to Maintenance Care once it's stable.

Active Care
$129 / month · first 4 months
  • Secure portal messaging with your clinician
  • Structured symptom updates & medication response review
  • Medication adjustments when clinically appropriate
  • Prescription & refill management
  • Lab guidance & review when needed
  • Up to 2 video visits included

The first four months — when symptoms, medications, labs, and dosing need the closest review. Additional patient-requested video visits $50.

Maintenance Care
$89 / month · month 5 onward
  • Everything in Active Care
  • Ongoing prescription & refill management
  • Dose review when symptoms change
  • Lab review when needed
  • Safety monitoring & annual care review
  • 1 video visit per year included

Once your plan is stable, your membership steps down automatically. Month-to-month — cancel with 10 days' notice. Additional video visits $50.

Your first-visit fee rolls forward — if you enroll within 2 weeks

The fee from your first visit — $50 video or $100 in person — is credited toward your first month of membership when you enroll within two weeks of your visit. So you pay just the difference on month one: $79 after a video visit, $29 after an in-person visit. That works out to less than $30 a week for unlimited access to your provider.

Add-ons are applied by your clinician when they're part of your plan — you don't select them here. They're added on top of your monthly membership.

Add-on · pairs with your membership
Testosterone Support
+$20 / month

For: patients whose plan includes testosterone. Covers the added monitoring, dosing, lab review, and controlled-substance refill oversight that testosterone therapy needs. Includes the required annual in-person testosterone review; an in-person visit is also required before testosterone can be prescribed.

Add-on · pairs with your membership
GLP-1 Support
+$75 / month

For: patients using or starting GLP-1 therapy (semaglutide or tirzepatide). GLP-1 care needs closer ongoing management — dose titration, side-effect support, lab monitoring, and refill coordination — so it's offered as an add-on rather than bundled in. It's available only after your hormone therapy is established and a medical assessment confirms it's appropriate.

  • Dose titration and escalation review
  • Side-effect guidance and management
  • Lab guidance and monitoring
  • Refill and pharmacy coordination
  • Metabolic and progress check-ins
  • Messaging with your clinician about your GLP-1 plan

Medication is not included. Your GLP-1 prescription is sent to the manufacturer's direct self-pay program, where brand semaglutide and tirzepatide are now available at substantially reduced cash prices, and you pay the manufacturer for the medication separately. This add-on covers your clinical management — the ongoing part insurance doesn't reimburse.

Step One
of two

Submit your enrollment form.

This is the form that officially registers you for membership — start here, before anything else. It takes about 5–8 minutes — it walks through your registration, billing authorization, lab preferences, and a photo ID upload, one section at a time.

Two systems, five minutes total. This form — including your ID — is fully done once you submit it. Right after, you'll set up your account and payment in a separate patient portal.
One thing worth repeating
Enrolled within 2 weeks of your visit? Your visit fee ($50 video / $100 in-person) is credited toward month one — so you'd pay $79 or $29 first month instead of the full $129.
Have these ready

The form below asks you to upload a photo of your driver's license or state ID — have it ready. The next step asks for a payment method, so have that nearby too before you start Step 2.

Opens in a new tab. Once you're done there, come back to this tab to continue to Step 2.

Started but stuck partway? Email info@menopauselouisiana.com and we can see where you left off.

Click this once you've submitted the form above — it moves you to Step 2 here, but doesn't submit anything or notify us on its own.

02
Step Two
of two

Complete your portal setup.

Last step, and it's quick. One trip to the patient portal covers everything below — plan to do both while you're in there so nothing gets left half-finished.

Both are required to begin treatment

We can't send prescriptions to your pharmacy or release your treatment plan until your account and payment method are on file. Most patients finish this in under five minutes.

Opens in a new tab. Once both below are done, come back to this tab to finish.

You're fully registered.

Our team reviews your registration and sends it to your provider. Within 24 hours (Mon–Fri), a message from your provider arrives in your patient portal.

If you asked to speak with Crystal before starting, she'll message you within 24 business hours instead — everything below still applies once you're ready to move forward.

What to expect

Finish both steps and treatment begins within 24 hours (Mon–Fri).

Once you complete both steps above, our team reviews your registration and sends it to your provider. Within 24 hours (Mon–Fri), a message from your provider arrives in your patient portal with everything you need to start.

  • Your personalized written treatment plan
  • Prescriptions sent electronically to your pharmacy — usually the same day, though some plans start labs first
  • Lab orders, if labs are part of your plan
  • Patient education and clear next steps for follow-up

A note on timing. Most prescriptions are sent to your pharmacy the same day your plan is released. In some cases — like when your plan starts with labs, or a medication needs a baseline result first — your provider may order labs before prescribing. If that's your plan, your portal message will explain exactly what comes first and when. Submitted on a weekend? Your 24 hours starts the next business day (Mon–Fri).

Your next step — when that message arrives
  1. Go to the Member Hub on our website and click Patient Portal.
  2. Open and read the message from your provider.
  3. If it asks you something — like your lab preferences — reply right there in the portal. Your provider finalizes your plan once you do.
