Most ongoing care happens through messaging in the patient portal β no scheduling, no waiting room, no copays per visit. Video visits are available when you need them.
Refills, dose changes, side effects, lab results β message us in the portal whenever something comes up. No appointments needed for most things.
No appointment needed β message your concern in the portal and your provider replies with prescriptions, education, and clear next steps. Not a chatbot, not a nurse line.
When your provider writes a prescription, it's sent electronically to your pharmacy the same day. Pickup timing is up to your pharmacy.
Insurance reimburses for visits β the appointment itself. It doesn't reimburse the messaging, refill management, lab review, and dose adjustments that actually deliver your treatment between visits. Membership is what funds that ongoing care so your provider can give you real attention without the constraints of fee-for-service billing.

Once your plan is stable, your membership steps down automatically. Month-to-month β cancel with 10 days' notice. Additional video visits $50.
The fee from your first visit β $50 video or $100 in person β is credited toward your first month of membership when you enroll within two weeks of your visit. So you pay just the difference on month one: $79 after a video visit, $29 after an in-person visit. That works out to less than $30 a week for unlimited access to your provider.
Add-ons are applied by your clinician when they're part of your plan β you don't select them here. They're added on top of your monthly membership.
For: patients whose plan includes testosterone. Covers the added monitoring, dosing, lab review, and controlled-substance refill oversight that testosterone therapy needs. Includes the required annual in-person testosterone review; an in-person visit is also required before testosterone can be prescribed.
For: patients using or starting GLP-1 therapy (semaglutide or tirzepatide). GLP-1 care needs closer ongoing management β dose titration, side-effect support, lab monitoring, and refill coordination β so it's offered as an add-on rather than bundled in. It's available only after your hormone therapy is established and a medical assessment confirms it's appropriate.

Medication is not included. Your GLP-1 prescription is sent to the manufacturer's direct self-pay program, where brand semaglutide and tirzepatide are now available at substantially reduced cash prices, and you pay the manufacturer for the medication separately. This add-on covers your clinical management β the ongoing part insurance doesn't reimburse.

The portal is where every part of your care lives β provider messages, your treatment plan, prescriptions, and education materials.
When the portal opens
- Click New Sign Up β not the regular login fields.
- Use the same email you gave us at your visit so we can match your account.
- Create your password and finish the signup prompts.

Add your payment method.
Required so we can process your membership. Nothing is charged until your registration is reviewed and your membership is confirmed.
Stored in Optimantra's encrypted vault. We never see the full card number.
Upload your photo ID.
Required for prescribing. We can't send prescriptions to your pharmacy without verified ID.
Three things to get right
- Set Record Type to ID Card.
- Choose your driver's license or state ID photo.
- Click Upload File before leaving the page.
The most common issue: patients choose the file but forget to click Upload File. Don't close the tab until you see the confirmation.

Once you complete the five steps above and submit your enrollment form, our team reviews it and sends it to your provider. Within 24 hours (MonβFri), a message from your provider arrives in your patient portal with everything you need to start.

A note on timing. Most prescriptions are sent to your pharmacy the same day your plan is released. In some cases β like when your plan starts with labs, or a medication needs a baseline result first β your provider may order labs before prescribing. If that's your plan, your portal message will explain exactly what comes first and when. Submitted on a weekend? Your 24 hours starts the next business day (MonβFri).
- Go to the Member Hub on our website and click Patient Portal.
- Open and read the message from your provider.
- If it asks you something β like your lab preferences β reply right there in the portal. Your provider finalizes your plan once you do.
Why the portal? All clinical communication lives in your patient portal β it's private and secure, so your treatment plan and your provider's replies are always there. You may get a text letting you know a portal message is waiting, or a quick supportive reminder along the way β but anything clinical, and any reply to your provider, happens in the portal.
