One step at a time.

This guided page walks you through each required step. Your provider cannot begin ongoing care until all 4 steps are complete.

You are fully registered when:

✓ Payment method added
✓ Photo ID uploaded
✓ Membership form submitted
Step 1

Create your portal account.

This is where all communication with your provider happens.

Important: Click New Sign Up — do not use the regular login fields unless you already created your account.

Step 2

Add your payment method.

Your card is required to activate membership care. Nothing is charged until your registration is reviewed.

In the portal, go to:
Demographic DetailsCredit Card On File
Open Portal →
Step 3

Upload your photo ID.

This is the step most commonly missed. Your registration cannot be processed without your ID.

In the portal, go to:
Documents & HandoutsID CardUpload File
Use a driver's license or state ID. Make sure the Record Type is set to ID Card.

Step 4

Submit your membership form.

This confirms your membership tier and officially activates ongoing care.

Final required step: You are not fully registered until this form is submitted.

You're fully registered.

Your provider will message you through the patient portal within 24 business hours, Monday through Friday.

Choose the tier your provider recommended during your visit. If you are still unsure, submit the registration form with your best choice and message us — we can help before processing.

What happens after registration?

Your provider reviews your completed registration and sends your personalized treatment plan, prescriptions, education, and next steps through the patient portal within 24 business hours.

I already created a portal account. What now?

Log in to the portal and continue with Step 2: add your payment method, upload your photo ID, and submit the membership registration form.
