Create your portal account.
This is where all communication with your provider happens.
Add your payment method.
Your card is required to activate membership care. Nothing is charged until your registration is reviewed.
Upload your photo ID.
This is the step most commonly missed. Your registration cannot be processed without your ID.
Submit your membership form.
This confirms your membership tier and officially activates ongoing care.
You're fully registered.
Your provider will message you through the patient portal within 24 business hours, Monday through Friday.

What happens after registration?
Your provider reviews your completed registration and sends your personalized treatment plan, prescriptions, education, and next steps through the patient portal within 24 business hours.
I already created a portal account. What now?
Log in to the portal and continue with Step 2: add your payment method, upload your photo ID, and submit the membership registration form.
