How to upload a document
like labwork
5 steps, about 30 seconds. Each screenshot below is the whole screen exactly as members see it — the red arrow points at the one thing to click.
On the Member Hub, click “Log in”
Find the tan Patient Portal card and click Log in at the bottom of it.
Sign in, then click “Log in”
Make sure Patient login is selected up top. Type your email and password, then click Log in.
Click “Documents & Handouts”
This link is always in the left-hand menu. If a setup form pops up first, close it or skip past it — you'll still see this menu.
Click “Choose File”
Set the Date of Record and Record Type above it first, then click Choose File and pick your labwork from your device.
Click “Upload File”
This sends the file. It'll appear under Your Documents right on this page once it's done.
Where it goes: uploaded files land under Documents & Handouts and are visible to Crystal on the provider side — no separate fax or email needed.
