Just like at any doctor's office, we can't see you until your paperwork is complete — the only difference is that here, the clipboard comes to you before your visit instead of in a waiting room. There are two separate forms below: the consent makes it legal for us to see you, and the intake gives us your history so your full 60 minutes go to you — not to filling out forms. Both must be submitted at least 24 hours before your appointment — completing one is not enough. If both forms are not in on time, your visit will need to be rescheduled.

How to join your appointment.
- You'll receive your video link by email the day before
- Log in a few minutes early
- Test your camera, microphone, and Wi-Fi ahead of time
- Use a quiet space if possible

- Enter through the front door on Camp Street and have a seat in the lobby
- Crystal will come down to greet you and bring you upstairs
- If you arrive early, feel free to relax — Crystal will come get you at your scheduled time

If you choose to move forward, treatment can typically begin within 24 hours of completing membership registration. Prescriptions and lab orders are sent promptly once enrollment is complete — no weeks of waiting.
